Eleanor Health welcomes community members who reside in the following states. We provide state-wide online appointments, with in-person services available at select locations.
If you have a commercial insurance and we are not in network with your payor, the answer is yes. However, if you are eligible for Medicaid or Medicare, you will not be able to participate. Call us or submit a form to discuss your specific situation.
Yes you can enroll your loved one into our self-pay program.
Cost of medication itself is not currently included. Our self-pay program covers live sessions with a medical provider, prescriptions and ongoing monitoring.
At this time we offer one self-pay program that specifically offers MAT (medication for addiction treatment).
There are no hidden fees or additional charges with our program.
There is no minimum commitment required but majority of our members stay with us for 12 months+ in our MAT (medication for addiction treatment) program.
A credit card is required to pay for our services and we ask you to store a card on file. The first month will be paid when you are setting up your first appointment. Going forward, your card will be automatically charged each month.
We strongly recommend that you discuss the end of your care with your provider so that they can provide you with the right guidance for transitioning out of care. If you wish to end our care, you can contact us at any time and we will process your cancellation and end future payments.
