We know a change in care can feel unsettling, especially if you’ve found something that’s working. At Eleanor Health, we start by understanding where you are today, what’s been working, what matters to you, and what you need next, so your care can continue without unnecessary disruption.
All available virtually, so you can get care from home.
It takes about 2–3 minutes. No commitment to talk.
We start by understanding what you’re currently taking and what’s been working for you. Your prescriber will review your current medication with you and work to maintain stability whenever it’s clinically appropriate. We’ll talk through any changes with you before anything shifts. When you schedule, bring your current medication information so your prescriber has the full picture. If you have questions about your current dose, you can talk them through with our team when you schedule.

No. You don’t have to re-explain everything from the beginning. Your first appointment is a continuation of care — we focus on understanding what’s already working and go from there.
This comes up a lot, especially if you’ve settled into a rhythm that works for you. If you’ve been seeing a provider on a schedule that’s keeping you stable, that’s important information, and we take it into account. Visit frequency is set with you at your intake and reviewed by your care team based on what you need, so we can’t lock in a specific schedule before then. In most cases we see people back weekly until we get their first drug screen results and then decide frequency from there.
Medication continuity is our priority. Your prescriber will review what you’re currently taking and work to maintain your regimen where it’s clinically appropriate. Bring your current medication information to your first appointment so your prescriber has the full picture.
Setbacks can happen, and they don’t cost you your care. Eleanor doesn’t remove members from the program because of a return to use. We’ll keep working with you.
A positive drug screen does not mean your care ends. In fact, the opposite. Eleanor Health does not discontinue care based on a positive drug screen. We will respond to your positive drug screen with increased support. We’re here to support you through the full picture of your recovery.
Eleanor accepts Medicaid and Medicare plans across our service areas and many major commercial insurance plans. We verify your insurance at no cost — call us and we’ll take care of it.
Most of your care happens virtually, so you can get support from home in the states we serve. In some states, you can also ask to be seen in person. If that’s what you need, call us and we’ll help you find the right option.
You’ve already taken important steps. We’re here to help you keep going.
