Finding the right support system is crucial for long-lasting recovery and overall well-being in mental health and addiction. At Eleanor Health, we understand the complexity of these challenges and provide comprehensive care that goes beyond traditional methods.

In this blog, we will introduce you to Eleanor Health’s care model and team roles. We’ll shed light on how they work together to provide holistic support for those seeking recovery and improved overall health.

Mental Health And Addiction Treatment

Eleanor Health offers a wide range of services designed to address mental health and addiction issues. Our mental health services encompass:

  • Psychiatric medication management
  • Therapy
  • Peer support

We believe that true healing requires addressing all dimensions of your life, and our multifaceted approach aims to support your overall well-being.

Understanding Whole-Person Care

At Eleanor Health, we embrace the concept of whole-person care, which involves looking beyond individual symptoms and considering the entirety of a person’s life experiences. Whole-person care is based on the understanding that each individual has unique needs and that optimal health outcomes are achieved by addressing all aspects of their lives.

Whether it’s finding stable housing, securing employment, or reducing alcohol intake, our care team is here to guide and assist you on every step of your journey.

Social Determinants Of Health

We recognize that social determinants of health  – defined by the World Health Organization as “the non-medical factors that influence health outcomes” – play a crucial role in a person’s overall well-being. These factors can significantly impact one’s ability to achieve and maintain good health:

  • Housing
  • Finances
  • Transportation
  • Access to resources

That’s why at Eleanor Health, we have dedicated professionals who specialize in helping you navigate these social barriers.

Community Recovery Partner (CRP)

A crucial member of the care team at Eleanor Health is the Community Recovery Partner (CRP). CRPs have lived experience with addiction and provide recovery coaching and support. They serve as mentors, helping individuals navigate their recovery journey. CRPs bring a unique perspective to care and play an essential role in breaking down barriers to success.

Therapist

At Eleanor Health, community members have access to various forms of therapy and counseling, including:

  • Individual therapy
  • Group counseling
  • Couples/marriage therapy
  • Family therapy

The therapists create a safe and non-judgmental environment, enabling individuals to process thoughts and emotions effectively and enhance their overall quality of life. Therapists use evidence-based psychotherapies like Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT) to help individuals:

  • Understand themselves better
  • Develop effective coping strategies
  • Work through mental health concerns

Psychiatrist

For individuals with more complex needs, such as co-occurring conditions or severe and persistent mental illness, the care team includes a psychiatrist. Psychiatrists specialize in treating mental health disorders with medications and assessing their appropriateness. They work closely with the care team to develop care plans and support therapy goals.

Psychiatrists play a vital role in medication management, ensuring appropriate prescriptions for enhanced response and symptom control. At Eleanor Health, the addiction medicine team manages anxiety, depression, and the complex relationship between mental health and substance use disorders.

Nurse Care Managers

Accessing both physical and mental health care in one place is a key advantage of Eleanor Health. Nurse Care Managers play a crucial role in helping individuals understand the available services and coordinate their care. They provide guidance on:

  • Accessing physical health care services
  • Offering education on chronic illnesses like diabetes
  • And ensuring that individuals’ health needs are well-coordinated

Doctor/Provider

If your care plan involves Medications for Addiction Treatment (MAT), you will meet with a doctor or provider. MAT is a proven approach to managing cravings and withdrawal symptoms in substance use disorder. It may include medications like Suboxone for opioid addiction, or Acamprosate for alcohol-use disorder. The doctor or provider will discuss your care plan, recommend the appropriate medication, and closely monitor your progress. The goal of MAT at Eleanor Health is to help you:

  • Manage your addiction
  • Adjust to treatment
  • Achieve stability
